OUR STORY
AIMAC exists because Australia’s health financing system is widely misunderstood – including by those required to use it every day.
The organisation emerged from Synapse Medical Services, an Australian software and advisory company specialising in healthcare payment integrity. Synapse was founded by Dr Margaret Faux, a nurse-turned-lawyer with more than 40 years’ experience across health systems.
Dr Faux is a recognised expert in Medicare and medical billing law and practice. She holds a PhD in medical billing compliance, has spent decades working at the intersection of law, policy, data and clinical practice, and has personally processed millions of medical bills. Her work is driven by a single concern: what happens when health-financing rules are misunderstood, misapplied, or ignored — by clinicians, regulators, or institutions.
Frustrated by the lack of legally accurate education about how Australia’s health system actually operates, she established AIMAC to deliver practical, defensible education on medical billing law, Medicare compliance, and the codes of conduct — grounded in legislation, case law and regulatory reality.
Our short, densely packed, fast-paced courses are entirely quiz-based, with immediate feedback that focuses on the application of relevant laws, legal concepts, and real-world takeaways. All courses are written by lawyers Margaret Faux and Bruce Topperwien, who review them regularly to ensure currency.

THE STORY
AIMAC’s flagship service, MBS Answers, was born in March 2020 at the start of the COVID-19 telehealth rollout. Within days, clinicians began asking urgent billing questions — and the volume never slowed.
For the first time, clinicians had a place to ask medical billing questions anonymously and receive legally accurate answers shared openly so others could learn.
During the pandemic the service was provided free of charge.
By 2023, the growing volume and complexity of questions — and the constant evolution of Medicare law — required a sustainable model. Answers needed ongoing review, updating and legal oversight. MBS Answers therefore transitioned to a modest paid subscription to ensure accuracy, independence and continuity.
Today, MBS Answers delivers answers that replaces folklore and opinion with law, clarity and accountability — for all health professionals and the future integrity of Medicare.

HOW IT WORKS
Subscribers can submit a question through the online portal. Questions are answered on a first-come, first-served basis and new answers are published in our weekly email update to all annual subscribers.
If your question is urgent or relates to an active dispute, audit, or investigation, you should seek independent legal advice. MBS Answers is designed to provide authoritative guidance, not real-time crisis management.

WHO WRITES THE ANSWERS?
All answers are written by lawyers, Margaret Faux and Bruce Topperwien, with every response independently reviewed and cross-checked by both authors.
Margaret Faux
Dr Margaret Faux is a nurse-turned-lawyer and one of Australia’s leading experts on Medicare and health insurance law. She has worked across the health system for over 40 years, including hands-on administration of millions of medical claims. She holds a PhD in medical billing compliance and is the author of How to Avoid the Medical Bill Rip-Off! a consumer guide to Medicare, private health insurance, and hidden healthcare costs. Margaret regularly provides expert evidence and advice in legal matters involving the interpretation of Medicare law and MBS items.
Bruce Topperwien
Bruce Topperwien is a former General Manager and General Counsel of the Professional Services Review Agency (the Medicare Regulator) and one of Australia’s most experienced Medicare law specialists. He has advised governments, insurers, hospitals, and legal practitioners on Medicare compliance and is the author of Health Insurance Act – Cases & Commentary, the leading legal text on Medicare billing law. Bruce brings deep regulatory, statutory, and case-law expertise to every answer published on MBS Answers.

HOW ANSWERS ARE FRAMED
Where the law is clear and settled, answers will be direct and unambiguous — often a simple “Yes” or “No” (for example, the bulk-billing signature requirements, which apply universally).
Where the law is conditional or fact-dependent, we will clearly state any limitations using language such as:
“Yes, except where…”
“In our view, no — unless…”
Each answer also specifies who it applies to. For example, supervision rules relevant to oncologists may not apply in the same way to pain specialists.

KEEPING INFORMATION CURRENT
Medicare law changes frequently. We endeavour to update answers promptly when significant changes occur, but we cannot guarantee same-day updates.
Each answer clearly displays the date it was last reviewed or updated, and subscribers are encouraged to check back regularly.

WHAT THIS SERVICE IS (AND ISN'T)
This service is intended to address complex medical billing and compliance questions.
We do not answer basic or administrative queries such as:
“Is there an item number for this?”
For those questions, consult the Medicare Benefits Schedule directly or contact Services Australia.
We do not provide business advisory services through this forum. For advisory services, please contact Synapse here.
We also assume subscribers have a working knowledge of Medicare fundamentals. Questions about basic principles will not be answered.
